MHE Incident Reporting Portal

Transform incidents into insights

There is currently no reliable national data on incidents involving material handling equipment. This is a challenge that has hampered efforts to collect accurate information regarding the true nature of forklift accidents. Without your stories, the industry cannot make any meaningful improvements – and that’s where our brand-new MHE Incident Reporting Portal comes in.

Open to everyone who works in the material handling industry, this platform provides a simple, confidential way to share experiences and help improve safety across the industry. It gives you the opportunity to tell us about any forklift incidents you have witnessed in total anonymity. By contributing, you will be playing a key role in making our sector safer.

A brief guide to using the portal

Filling out the form is a straightforward, secure process – all that is required of you is to submit as much information as possible.

Before you get started, please remember the following:

  • Be sure to include a time, date and partial postcode to help avoid duplicates
  • Refrain from including specific names of people and companies
  • All data submitted to the portal is anonymised and used only for the purpose of analysis
  • All reports are reviewed by UKMHA staff with the utmost privacy
  • This is not a replacement for RIDDOR reporting, and no follow-up action will be taken (unless specifically requested)
You can report anything from near misses and minor damage to serious accidents – every detail helps, regardless of how minor you consider it to be.

If you have any questions or wish to add to or amend details for a previously submitted report, then please contact accident@ukmha.org.uk.
