Reporting through this portal does not replace statutory reporting duties. Reporting of some injuries, diseases and dangerous occurrences can be a statutory obligation. For more information visit https://www.hse.gov.uk/riddor/reporting/index.htm
There are no reliable incident/accident statistics for MHE in the UK because HSE does not record or analysis RIDDOR data by equipment type. Effective safety improvements cannot be implemented without information about incidents; the stories of users.
The purpose of the portal is to identify targets for safety gains and ensure our efforts are effectively targeted; there will be no enforcement action and no follow up activity unless this is specifically requested.
An email address is required for verification purposes only.
This portal should not be used for reporting incidents involving MEWPs (Mobile Elevating Work Platforms), MCWPs (Mast Climbing Work Platforms) and Construction Hoists (CHs). These incidents can be reported to IPAF at https://ipafaccidentreporting.org/
Reports will not be published directly on the website and no running commentary will appear
Reports will be reviewed internally by UKMHA staff so that trends, common factors and priority items can be identified
Analysis of the data will be published and made publicly available and redacted data may be shared with third parties
No identifiable personal information or company data will be shared with third parties
Please do not include any personally identifiable data when reporting. This includes site/company name, names of persons, brand of equipment, etc. Any information of this type included in reports will be redacted before publication or sharing with third parties.
Please note that time, date and partial postcode information is important to enable eradication of duplications.
The portal is suitable for reporting near miss and minor incidents and incidents involving only damage to equipment, goods or structures, as well as serious accidents
Please provide all detail available and, when possible, your own perspective of how the incident occurred and the causes, however, we appreciate that it is not always possible to complete all of the fields or to identify causal factors.
The portal can be used for reporting incidents individually, for example as they occur. There is an option to indicated that updated or additional details are being added to an earlier report.
In the event that you wish to submit reports in bulk then this can be done by emailing a csv file to accident@ukmha.org.uk using this csv template.
